- Service you are seeking (Here is where you specify is you are looking for a fetish session, FMTY, dinner or social dates, etc. If none apply, please specify only the duration of time you are looking to meet.)
- Date, time and duration of meeting.
- Provide necessary screening requirements.
In regards to my booking form, I require ONE of the following forms of screening to meet:
- Government ID
- Work Badge, Linkedin, or Business Card
- P411/PD accounts (Active reviews and OK’s)
- 2 provider references (within the past 2 years)
